The Role
You’ll:

• Assess and review children and young people for NHS Continuing Care.
• Manage cases and create tailored care plans.
• Collaborate with health, education, and social care teams.
• Lead multi-agency panels reviewing care eligibility.
• Support personal health budgets and national care frameworks.
• Inspire service improvements and your team.
