We are recruiting for a Heart Failure Nurse Specialist to join #TeamIOWNHS. You should share our passion and drive to ensure that our patients and staff receive quality and compassionate care and work with us to ensure the Trust continues its journey towards great people, great place.

As part of a highly developed integrated, nurse-led heart failure team, you’ll help ensure the service standards meet the requirements of the National Institute of Clinical Excellence (NICE) guidelines CG187 & NG106.

You’ll provide evidence-based specialist nursing assessment, advice to patients with a confirmed diagnosis of Heart failure with either a preserved ejection fraction (HFpEF) or a reduced ejection fraction (HFrEF) and support to nurses in the acute or community setting.

Working closely with St. Marys colleagues (cardiac, elderly, general medicine and palliative teams), CCG colleagues, General Practices, local hospice and Tertiary centres, you’ll help in providing and developing a service that will meet and maintain standards across primary/secondary care pathway.
