Great Chapel Street, a homeless health specialist GP service have an exciting opportunity to work in a multidisciplinary team in Inclusion Health. The role is based both in clinic at Great Chapel Street and in outreach at hostels in Westminster. During outreach you would be an important part of the team running a project called 'Health Beds'. You would work alongside an ANP supporting patients for an 8 week period to provide intensive and holistic health care. This project makes a huge difference to patients’ lives and whilst it is complex and challenging, it is incredibly interesting and rewarding. This patient group requires creativity, advocacy and flexibility; therefore our service strives to meet this.

This post would be part of a really supportive, friendly team with the opportunity to work in a truly holistic way alongside a large spectrum of practitioners and teams. You would be able to work autonomously, using your skills and knowledge to create the best possible outcomes for our patients.

The role is Monday-Friday normal working hours with a firm respect for work/life balance. The challenges of working with such complex patients is well recognised and supported with regular supervision. Whilst experience in inclusion health would be brilliant, this role would also suit someone interested in the field and able to bring a different set of skills to the role from other work backgrounds.
